This illustration depicts injuries and surgical interventions in the left knee. The anteromedial view reveals grade IV osteochondritis between the patella and femur, intercondylar notch fraying, a lax anterior cruciate ligament (ACL) with partial cyclops lesions, and a tear of the posterior horn of the medial meniscus. The surgical process begins with an incision made on the superomedial aspect of the knee, accompanied by arthroscopic visualization, allowing evacuation of yellow or clear joint fluid. Photothermal ablation is then utilized to shrink the ACL, followed by a notchplasty to widen the intercondylar space and create additional room for the ligament. Abrasion arthroplasty is performed to address cartilage defects located on the posterior surface of the patella.
