This illustration shows the pre-operative condition and surgical procedure for ACL reconstruction in a knee with multiple injuries. The pre-operative view reveals an attenuated ACL with torn fibers, adhesions causing a lateral tilt of the patella, and a re-tear in the previously operated lateral meniscus. The surgical procedure begins with the insertion of arthroscopic instruments (A), followed by debridement of the meniscal tear and smoothing of cartilage surfaces (B). Adhesions are lysed to realign the patella (C). A bone-ligament graft is harvested (D), and drill holes are created in the femur and tibia to anchor the graft (E). The graft is then pulled through these tunnels and secured with screws to replace the torn ACL (F).
